Humana is seeking a Case Manager to serve members in the Wisconsin Family Care program. You will assess health and safety needs, develop a comprehensive Member Care Plan (MCP), and coordinate with a Field Care Nurse to support members in the least restrictive environment.
The role requires a Bachelor’s degree in human services or related field (or 3+ years of experience with another degree), valid driver’s license, up to 40% travel, and hours Monday–Friday 8:00 a.m.–4:30 p.m. CST.
