Description

Do Work That Changes Lives.

What if your next job wasn’t just a job—but an opportunity to help someone build a more independent and fulfilling life?

Family & Children’s Center is seeking a compassionate, motivated Case Manager to join our Community Support Program (CSP).

In this role, you’ll work directly with individuals living with severe and persistent mental illness, helping them navigate challenges, connect with resources, develop support systems, manage symptoms, and work toward their personal recovery goals.

You’ll build relationships. You’ll solve problems. You’ll advocate. And you’ll see the impact of your work.

Why You’ll Love This Role

Purpose-driven work
Help people achieve recovery goals and successfully live in their communities.

Relationship-centered care
Build meaningful professional relationships with clients and collaborate with families and treatment providers.

Community-based work
Get out of the office and work directly with clients in their homes and communities.

Every day is different
No two clients—or two days—are exactly alike. You’ll use your skills, creativity, and compassion to respond to individual needs.

What You’ll Do

As a CSP Case Manager, you will:

  • Provide mental health case management services to individuals with severe and persistent mental illness.
  • Complete assessments and create treatment, recovery, and crisis plans.
  • Help clients identify and achieve their individual recovery goals.
  • Provide strengths-based, recovery-oriented services.
  • Help clients manage psychiatric symptoms and co-occurring medical and substance use concerns.
  • Provide symptom monitoring, education, crisis intervention, and medication education/monitoring as directed.
  • Connect clients with community resources related to housing, food, healthcare, safety, and independent living.
  • Coordinate care with internal and external treatment providers.
  • Engage family members and support systems when permitted by the client.
  • Participate in treatment meetings and ongoing supervision.
  • Maintain accurate, timely clinical and case management documentation.
  • Provide after-hours crisis services by phone or face-to-face when needed.
  • Assist with discharge planning and referrals.

This is a community-based position, with at least 50% of the work occurring in community settings or clients' homes. Local travel is required.

Qualifications

What We’re Looking For

You might be a great fit if you are someone who:

  • Builds trust easily and treats people with dignity and respect
  • Believes people can achieve meaningful recovery
  • Is compassionate, dependable, and organized
  • Enjoys working directly with people rather than sitting behind a desk all day
  • Can balance independence with teamwork
  • Communicates effectively with clients, families, providers, and coworkers
  • Is comfortable responding to changing needs and crisis situations
  • Understands the importance of confidentiality, ethics, and professional boundaries
  • Committed to recovery-oriented and strengths-based services

Required Qualifications

Education & Experience:

  • Bachelor's degree in behavioral science or a related field plus 1,000 hours of supervised post‑degree clinical experience with persons with chronic mental illness; OR
  • Bachelor's degree in another field plus 2,000 hours of supervised post‑degree clinical experience with persons with chronic mental illness.

Candidates should also have familiarity with the human service delivery system, DHS 63, recovery principles, ethics, and professional boundaries.

A valid driver's license and ability to use your own vehicle for client transportation are required.
