- Provide 24‑hour supervision of individuals in service primarily in the home setting and the community, as well as day habilitation/program sites and work sites, as needed.
- Work in a Personal Care Alternative setting or Group Home setting, as needs require.
- Train and teach individuals in activities of daily living and work toward their highest potential while maintaining independence.
- Maintain program files and assist in achieving program goals to maintain accreditation.
- Teach individuals in accordance with the Individual Plan (IP) and the Individual Services and Support Plans (ISSP), increasing independence and social skills.
- Promote reasonable and appropriate supervision for individuals in service in accordance with the IP.
- Pass the Medication Administration test with no more than two attempts and administer medication according to physician orders and appropriate techniques.
- Respond to medical and other emergencies appropriately and communicate health concerns to the agency’s nurse or other designated personnel.
- Assist in cooking, cleaning, personal hygiene, and incontinence care; provide light snow removal, vehicle cleanliness, and related tasks.
- Perform lifting and transferring techniques as trained, lifting up to 100 pounds (floor to waist, waist to shoulder, or shoulder to overhead).
- Drive agency vehicles (and personal vehicles) as necessary for transportation of individuals in service and to perform other agency business.
- Regularly sit, walk, stand, climb stairs, and restrain individuals pursuant to training; sit at a computer for long periods of time and repeat hand, arm, or finger motions many times.
- Communicate through speech with individuals in service and the public in both quiet and noisy environments.
- Operate a personal computer and related software.
- Tolerate various levels of stress and meet deadlines with severe time constraints.
- Demonstrate specific vision abilities: close, distance, and depth perception and read handwritten or typed material.
- Work multiple sites or work flexible schedules as agency and supervisor deem necessary; staff will be required to work irregular, extended, and/or be available for on‑call work hours and may work evenings, nights, weekends, holidays, overnight shifts and remain awake for up to 16 hours as individual needs require.
- Assist with the development of annual assessments and Individual Service and Support Plans and track the programs as indicated for assigned individuals.
- Safeguard all agency funds and property; plan and implement recreation activities and provide community access for individuals.
- Follow agency policies and procedures in carrying out duties, perform other duties as assigned by supervisory staff and/or designee, and perform other reasonable duties as may be assigned.
- High school diploma or GED; must be at least 18 years of age.
- Licensed drivers with reliable transportation; current Colorado driver’s license within ten days of hire if out of state.
- Background check, drug and alcohol check, reference check, and motor vehicle checks passed.
- Current motor vehicle insurance required; driving is an essential job duty.
- Six months experience working with individuals with developmental disabilities (preferred).
- Must have passing scores on Medication Administration test; ability to lift up to 100 pounds with proper technique.
- Must be able to serve as a direct and supportive caregiver in a person‑centered environment.
Regularly sit, walk, and stand. Occasionally twist at the neck and/or trunk, squat/stoop/kneel, reach above the head, and perform forward motion. Must repeat the same hand, arm, or finger motion many times. Manual and finger dexterity essential. Must lift and/or push and pull up to 100 pounds. Lifting may require floor to waist, waist to shoulder, or shoulder to overhead. Tolerance for various levels of stress required.

Noise level usually moderate to loud. Working conditions include inside and outside environments. Occasionally work irregular or extended hours and meet deadlines with severe time constraints. Employee is directly responsible for the safety and well‑being of individuals.
