The Licensed Vocational Nurse/Wellness Coach provides case management for an assigned group of clients including coordinating and monitoring the activities of the treatment plan. The incumbent will complete and oversee assignments to improve the diet, nutrition, and wellness of the clients.
LICENSED VOCATIONAL NURSE (LPN) RESPONSIBILITIES:Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job.
- Assumes an active role in the treatment team, providing appropriate input as needed.
- Completes all charting and documentation in the electronic health records as contractually required and in timely manner as dictated by program requirements.
- Coordinates medication orders and refills with pharmacies.
- Responds promptly to client calls regarding medication issues.
- Conducts comprehensive assessment of psychiatric history and physical health needs as part of the treatment team.
- Provides client education on medications and their side effects.
- Collaborates with Psychiatrist and Registered Nurse to monitor client wellbeing and medication compliance along with medication efficacy and side effects.
- Responds appropriately to medical emergencies.
- Takes and records vital signs.
- Initiates appropriate and timely interventions on an ongoing basis and provides crisis intervention as needed.
QUALIFICATIONS:
- Vocational Nursing diploma from an accredited program.
- Current licensure as a Vocational Nurse in the State of California.
- At least three years of experience treating psychiatric population.
- Current California license as a vocational nurse.
- Bilingual, Spanish speaking if the position serves Spanish speaking population.
- Experience working with psychiatric and dually diagnosed populations preferred.
- Proficient user of Microsoft Office Suite and EHR software.
- Ability to problem solve and react appropriately to emergency situations.
- Ability to work independently and effectively collaborate with a team.
