Home Health / Hospice :The CWOC RN will administer skilled nursing care to patients vides care for individuals who have, or will have, a diverting ostomy. Provides wound and skin evaluation and care. This specialized nursing care is given independently and in collaboration with other members of the healthcare team under the direction of and in accordance with the physician’s orders. Functions as an educator for patients / significant others and staff.

- Able to establish effective, timely working relationships with patients, families, staff and physicians.
- Expertise in the nursing process in order to develop an individualized patient plan of care.
- Clinical competence, including demonstrable skills in the establishment of peripheral intravenous lines and administration of intravenous therapies.
- Computer skills.
- Principles and methods of adult education
- Graduate from accredited school of nursing
- BS degree
- Minimum of two (2) years clinical experience in acute care setting
- Home Health experience preferred.
- Current California RN license.
- Current CPR card.
- Current California driver’s license and valid auto insurance.
- Wound, Ostomy, Continence Nurse Certification.
- BSN or MSN degree preferred
