• Implement person‑specific interests and likes in community activities.
• Monitor and evaluate interests and needs.
• Assist in developing and achieving outcomes in individual personalized plans.
• Document progress of goals and additional support measures.
• Plan community engagement activities that match individual interests.
• Plan activities that expand community relationships and social skills.
• Participate actively in all aspects of person support.
• Coordinate community supports five days per week.
• Provide support while at the person’s workplace.
• Identify potential employment opportunities in the community that match interests.
• Assist in the discovery process to identify passions and interests related to meaningful activities and employment.

• High school diploma or equivalent (GED).
• Ability to lift 50 lbs. or more.
• Clean driving record with at least 2 years of driving experience.
• Basic computer skills.
