Hospice

• Participates in the coordination and delivery of patient care according to the professional role of a licensed Nurse Practitioner, providing clinical care, patient visits, examination, diagnosis, and treatment.
• Provides education to patients, families, staff, and community as required, and coordinates, conducts, and participates in evaluation of Nurse Practitioner services.
• Implements evidence‑based practices to improve patient outcomes and is available for consultation with Agency staff.
• Has a written Collaboration Agreement with a supervising Physician in compliance with community and state standards.
• Participates in interdisciplinary team conferences to review and plan patient care.
• Provides direct patient care and evaluation of palliative care patients in both home and clinic settings.
• Acts as a consultant to Physicians and other Nurse Practitioners on patient care issues, including medical interventions to relieve pain and symptoms of disease, and recommending appropriate medication, dosages, treatments.
• Documents orders and progress notes in patient chart promptly.
• Functions as an on‑call consultant to other team members regarding patients’ physical conditions.
• Provides education through teaching, in‑services, new employee orientation, and other interactions to enhance clinical competencies of the Agency’s staff.
• Serves as liaison between Agency, community physicians, and the medical community; participates in hospital and community education programs.
• Participates in routine record reviews and provides patient information to the Medical Director or other Physicians for eligibility determinations.
• Performs job functions using applicable computer technology; maintains medical records in compliance with documentation requirements and coding procedures.
• Gathers statistical data and prepares reports; participates in quality assurance, process improvement, and evaluation of patient care services.
• Participates in health‑related research projects as assigned and is committed to agency resolution against fraud and abuse.
• Is knowledgeable in hospice and palliative care philosophy, LCD, and state and federal rules and regulations.

• Thorough knowledge of nursing and medical policies and procedures related to practice area.
• Demonstrated commitment to Agency’s mission and values.
• Analytical skills necessary to assess patients and participate in appropriate follow‑up care planning.
• Interpersonal and communication skills (verbal and written) effective with patients, families, physicians, and healthcare professionals.
• Ability to perform physical tasks such as standing, walking, sitting, climbing stairs, stooping, kneeling, twisting, bending, reaching overhead and in front of body, simultaneous use of both hands, wrists, and fingers.
• Can operate and drive an automobile with a valid driver’s license and current automobile insurance; personal vehicle required.
• Will require participation in medical staff activities.

• Successful completion of an accredited Nurse Practitioner graduate program or an accredited certificate program.
• Current national certification as a Nurse Practitioner.
• Current and valid RN licensure with prescriptive authority in Indiana.
