Certification Details
- Conduct Home Health Nursing assessments to determine nursing, personal care, and equipment needs in the home.
- Regularly re-evaluate participant nursing needs.
- Initiate and revise plan of care as participant status changes.
- Develop care plans with goals based on nursing diagnosis including therapeutic, preventative, and rehabilitative nursing actions.
- Deliver and document home health nursing interventions as per care plans.
- Instruct participants, families, and caregivers on disease processes, self-care, and prevention strategies.
- Communicate changes in participant condition to interdisciplinary team.
- Manage care transitions between settings with Welbe Health Providers and team.
- Identify and implement discharge planning needs.
- Supervise and evaluate home health aide care every two weeks.
- Participate in end-of-life care coordination and support.
- Provide backup coverage to other Welbe Health nursing functions.
- Perform duties consistent with Nursing Practice Act and California Code of Regulations.
- Work varied schedule including evenings, nights, weekends, and overtime.
- Complete required documentation timely and accurately.
- Maintain confidentiality of company and participant information.
- Follow Welbe Health policies, participate in quality improvement, training, and meetings.
- Communicate regularly with supervisor regarding workload and priorities.
- Complete mandated trainings and occupational health screenings.
- Exercise flexibility as business needs evolve.
- Other duties as assigned.
Job Details
- Nursing knowledge and skills to treat frail, elderly participants and manage complex clinical situations.
- Ability to initiate venous access for IV hydration or lab draws; experienced with IM, SC, transdermal injections, and wound care.
- Highly motivated, self-directed, able to make sound decisions in emergencies.
- Excellent clinical, organizational, and communication skills with seniors, families, and interdisciplinary teams.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in fast-paced environment.
- Experience working with diverse backgrounds and cultures.
- Commitment to supporting vulnerable seniors.
- Minimum of two (2) years nursing experience in clinical setting with frail or elderly population preferred.
- Home health agency experience preferred.
- Graduate of an Accredited School of Nursing required.
- Unencumbered Registered Nurse (RN) License required, meeting home health agency qualifications.
Job Requirements
- Willingness and ability to work varied schedule including evenings, nights, weekends, and overtime.
Schedule Information
