As a crucial team member, you’ll facilitate referrals and ensure access to vital community resources while sharing your counseling expertise.
Key Responsibilities:
• Develop and implement comprehensive treatment plans
• Provide ongoing case management and support
• Lead assessments and collaborate with interdisciplinary teams
• Support crisis intervention and individual counseling
• Educate clients about available community resources
Requirements:
• Bachelor of Social Work (BSW) needed
• Eligible for registration with licensing organizations
• Strong counseling skills and community engagement knowledge
• Familiarity with addiction and trauma-informed methods
• Ability to work in diverse, community-focused environments
