Care Manager RN
The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Care Manager Navigator assists patients to gain self‑efficacy/management skills, achieve optimum functional health status, and quality of life.
Assists patients/families, staff and systems to achieve high quality, evidence based, cost‑effective, and patient focused outcomes.

Required Qualifications- Graduation from an accredited school of nursing
- Upon hire:
Washington Registered Nurse License - 2 years experience in RN role
- Bachelor's Degree in Nursing
- Experience in managing complex patients in an ambulatory setting
- Medical Home Model experience
