We are seeking a qualified Home Health RN for a 13‑week travel assignment in Lacey, Washington (ZIP 98503). This role involves delivering direct nursing care in patients' homes while working with a dedicated clinical team.

- One year of recent experience as an RN, including a minimum of six months in a Home Health setting
- Proficient in OASIS documentation
- Valid RN license in compliance with state regulations
- BLS certification (AHA/ARC)
- Home Health Nursing Certification (RN-BC)
- Other certifications and licenses may be required for this position
A Home Health RN delivers direct nursing care in patients' homes, ensuring comfort, stability, and effective management of medical conditions.
Essential Work Functions- Assess patient conditions and develop individualized care plans based on medical needs and physician directives
- Administer medications, treatments, and wound care while monitoring patient responses
- Provide patient and caregiver education on medication management, disease progression, and self‑care techniques
- Monitor for changes in patient status, reporting concerns and adjusting care plans as needed
- Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions
- Document patient care, interventions, and progress accurately and in compliance with regulatory standards
- Ensure compliance with infection control, safety, and home‑care best practices to protect patients and caregivers
- Support families and caregivers, offering guidance and emotional support throughout the care process
- Maintain communication with case managers and interdisciplinary teams to provide coordinated patient care
- Complete charting and documentation requirements while meeting visit and productivity goals
- Perform other duties as assigned within the scope of practice
- Critical thinking, service excellence, and good interpersonal communication skills
- Read, write, and communicate effectively in English
- Comprehend written and verbal instructions; PC skills
- Physical capability: remain stationary, move equipment (50–100 lbs), push, pull, bend
- Interpersonal skills: work effectively with diverse personnel, maintain a positive attitude
- Technical skills: fine manipulation, assist with procedures and equipment
- Mental resilience: manage frequent public contact, meet deadlines, work under supervision or solo as needed
