American Traveler is seeking a Home Health PTA with HCHB experience and 1+ years of recent home health experience to provide geriatric rehabilitation care.
Requirements- Minimum 1 year of recent relevant experience required
- BLS certification required
- OASIS experience required for staff completing Start of Care (SOC)
- Proficiency in HCHB required
- Active driver's license and personal transportation required
- License required if applicable
- Timely documentation and communication required
- Hospice/Home Health checklist required for consideration
- Home Health setting serving a geriatric patient population
- Caseload of approximately 5-7 patients per day
- Productivity expectation of 6 daily visits or a minimum of 30 points weekly
- Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
- HCHB (Homecare Homebase) EMR experience required
- Territory covers up to a 60-mile radius from the hub office, with occasional additional distance based on patient needs
- No on-call requirement, though rotation may be discussed during interview
- Provide rehabilitation care for geriatric patients including orthopedic, neurologic, and post-surgical conditions
- Conditions treated include hip fractures, joint replacements, spinal surgeries, stroke recovery, and traumatic brain injury
- Utilizes equipment such as gait belts, walkers, crutches, and upper/lower extremity prosthetics
- Scrubs required in any color; no jeans permitted
- No cell phone provided; iPad provided for documentation
- Mileage reimbursed at the IRS standard rate
- Monday orientation required, including completion of billable training modules
- First-time travelers will not be considered for this role
- Travel pairs accepted, though each traveler must maintain their own territory and transportation
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- Requested time off (RTO) must be submitted at the time of application
- Local candidates accepted at the travel rate
- Traveler must remain flexible with territory assignments based on patient needs
