Description of job: As a Licensed Practical Nurse (LPN), you will deliver compassionate, resident-centered nursing care in both short-term rehab and long-term care settings. You'll collaborate closely with interdisciplinary teams, including therapists, social workers, and nursing leadership, to ensure high-quality, individualized care plans. Your role supports recovery and functional independence, all while upholding the facility's standards of dignity and respect.

Evening shifts: 2:45 p.m.

-11:15 p.m., five shifts per week on an anticipated rotating two-week schedule.

• Current BLS certifications
• Current (NH) Nursing license
• 1+ year of experience in Skilled Nursing/LTC/Rehab experience

• Assess and monitor residents' health status, documenting vital signs, symptoms, and care responses accurately.
• Develop and implement carecare plans based on clinical assessments, including medication management and therapeutic interventions.
• Coordinate interdisciplinary care, facilitating communication between nursing, therapy, social services, and leadership to promote seamless resident care.
• Educate and support residents and families on health conditions, treatment options, discharge planning, and rehabilitation progress.
• Ensure regulatory compliance and uphold clinical standards, adhering to policies, safety protocols, and quality measures for optimal resident outcomes.
