TRU PRN PACE Social Worker at St. Paul's Senior Services
Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Schedule: PRN / Picking up as needed to cover PTO or sick time, within working hours of Mon‑Fri 8 am to 5 pm.
Location: Hybrid in Lafayette as needed for participant contact. Some in‑home visits may be required.
MSW/LSW Pay: $32.97 to $41.26 per hour
LCSW Pay: $35.27 to $44.11 per hour
The PACE Medical Social Worker plans, organizes, and implements social work case management services for PACE participants and families. Social work case management interventions include psychosocial assessment, resource/referral, coordination and facilitation of family and facility care conferences, discharge planning, long-term care and respite planning, advance care planning, advance directives, mental health/crisis assessment, support and referral, assistance with financials and access to benefits as needed, and pre-enrollment and disenrollment procedures.
Education and Experience- Master’s Degree in Social Work
- 2+ years relevant experience as a medical social worker
- Experience working with the frail or elderly population preferred
- Experience working with individuals with mental health needs preferred
- Experience working within an Interdisciplinary Team (IDT) model preferred
- Provides direct medical social work case management to a designated caseload of TRU PACE participants and families.
- Completes initial and ongoing reassessment of participant and family psychosocial needs based on emotional, environmental, financial, psychological, cognitive, social, cultural and health factors.
- Collaborates with the interdisciplinary team, participant and family in the development, implementation, evaluation, and revision of the comprehensive care plan.
- Evaluates quality of psychosocial interventions and adjusts interventions as clinically necessary.
