Seeking dynamic Therapy leader for Director of Patient Outcomes – Full Time – Lafayette, Indiana
Position Summary:
- The Director of Patient Outcomes (Rehab) is the primary individual responsible for implementing and coordinating the patient assessment and evaluation process for all patients treated by the staff in the Rehabilitation Department.
- Provides oversight and clinical support in completion of data contained in the PAI and Quality Indicator data for each patient to ensure accuracy in the scoring of functional status and reflection of patients’ care needs.
- Audits and oversees interdisciplinary and regulatory functions in clinical care.
- Facilitates process improvement and best practice by analyzing data and developing creative ways to improve performance.
- Direction and oversight of Case Management Department, including discharge planning, Length of Stay management, and transition to next level of care.

QualificationsRequired Skills:
- Clinical license required (PT, OT, SLP, or RN) in the state of employment required. Other licensure/certification to be considered based on appropriate qualifications.
- Two years recent experience in acute care setting preferred.
- Leadership/supervisory experience required.
