Primary Care Clinic

KEY RESPONSIBILITIES

  • Strengthens the connection between patients, healthcare teams, and social service systems by providing hands‑on support to individuals managing chronic conditions.
  • Working primarily in a clinic environment, engages patients in disease‑specific programs through education, coaching, and care coordination, while also identifying eligible individuals and facilitating enrollment into Remote Patient Monitoring (RPM) services for conditions such as hypertension, obesity, maternal health, and other targeted health needs.
  • Maintains consistent face‑to‑face interaction with patients and care teams through clinic huddles, outreach, and coordination activities, and collaborates closely with clinical staff to track progress toward care goals.
  • Conducts occasional home visits to assess barriers to care, reinforce self‑management skills, and ensure effective use of remote monitoring equipment.
  • By bridging clinical care and community resources, this role supports patient engagement, reduces obstacles to care, and helps improve health outcomes across the population served.

REQUIREMENTS

  • High School Diploma/GED is required.
  • Requires 0-3 years of relevant experience.
  • Must obtain a Community Health Worker certification within 1 year of hire, in accordance with state and organizational standards.
  • Ongoing continuing education may be required to maintain certification.
  • Requires basic computer skills.
  • Requires knowledge of medical, community, and social resources.
