Admissions Nurse PRN RN – Anew Hospice

Lafayette, IN

Summary

The Admissions Nurse determines a patient’s eligibility for hospice services, reviews medical records, completes in‑person evaluations, and works with the Medical Director to establish the initial plan of care.

Responsibilities
  • Completes an initial comprehensive assessment of patient/client and family to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illnesses.
  • Responsible for setting up DME and medications.
  • Provides professional and direct nursing care by utilizing all elements of the nursing process and the State Nurse Practice Act.
  • Counsels the patient/client and family in meeting nursing and related needs.
  • Provides health‑care instructions to the patient/client as appropriate per assessment and plan of care/
    nursing diagnosis

  • Communicates with the physician regarding the patient’s/client’s needs and reports any changes in condition; obtains/receives physician’s orders as required.
  • Ensures arrangements for equipment and other necessary items and services are available.
  • Provides education to patients and families regarding available hospice services.
  • Demonstrates C.A.R.E. values to patients/clients, family members, customers, and staff.
  • Complies with the company’s privacy practices/procedures and all state/federal privacy laws as outlined by HIPAA.
  • Demonstrates teamwork and maintains regular attendance to ensure quality care and services.
  • Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) as required by the Bloodborne Pathogens Standards.
Qualifications
  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute‑care experience in an institutional setting.
  • Current Registered Nurse license in the state and CPR certification.
  • Bachelor’s degree; one (1) year of home‑health care experience preferred.
  • Management experience not required; responsible for supervising home‑health aides.
