St. Joseph Hospice located in Lafayette, LA is seeking a Full-Time Clinical Liaison (RN) to join our team.

We are seeking a candidate with a strong clinical background, payer knowledge, and great communication skills.

The Clinical Liaison (RN) will be responsible for working with physicians, case managers, and patients' families to assist in providing the best continuum of care for patients that require hospice care.

Requirements:

• Registered Nurse (RN) in the State of Louisiana with a minimum of 2 years’ experience
• Have a professional demeanor with excellent communications skills
• Be flexible, self-driven, accountable for results, autonomous, yet supportive of team efforts.
• Have a valid driver’s license, proof of valid auto insurance, and active clinical license are required.
• Prior sales or business development experience, preferred
• Hospice or Home Health experience preferred
• HCS Interactant software experience preferred, but not required
