The mission of Wellstar's Department of Spiritual Health is to enhance the spiritual health and well‑being of every person we serve. Our vision is to deliver world‑class spiritual care to every person, every time. We do this by impacting the healing experience through compassionate presence, hope and collaboration with patients, residents, families, staff and the community. As a clinically‑trained Chaplain (PRN), you will offer spiritual health care and emotional support to patients/residents, families and support systems from all faith traditions. You will perform spiritual assessments, develop individualized care plans, offer spiritual interventions, collaborate with the multidisciplinary team and provide beginning‑of‑life and end‑of‑life support, legacy work and bereavement care.
You will work closely with the faith community, create a network of support, educate team members on self‑care and compassion fatigue, and participate in the on‑call rotation as determined by site‑specific needs.
- Spiritual Health Care to Patients/Residents and their Families/Support Systems
- Provide spiritual, religious, and emotional care to patients, residents, families and their support systems, irrespective of faith tradition or spiritual path.
- Possess a working knowledge of major world religions and integrate patients' belief systems into plan of care decision making.
- Perform spiritual assessments to gather and evaluate the spiritual, religious, emotional, and social needs of patients/residents and their families; determine appropriate spiritual interventions.
- Act as advocate for the patient/family; interpret the impact of cultural beliefs on the plan of care.
- Offer emotional support and perform legacy work with patients nearing end‑of‑life; provide bereavement support to families.
- Facilitate advance care planning with patients/residents and their families/support systems.
- Document in EMR all spiritual health encounters and interventions.
- Collaborate with multidisciplinary team on the care plan to ensure spiritual health support and care; contribute to the well‑being of patients/residents and families/support system, including attendance at IDRs.
- Participate in on‑call rotation (including evenings, overnights, weekends, and holidays) on a regular basis, as determined by site‑specific needs; required of all chaplains.
- Hospice chaplains ONLY:
Participate in interdisciplinary team (IDT) meetings and contribute to the plan of care to address the spiritual needs of the hospice patient and family. - Hospice chaplains ONLY:
Determine an individualized plan of care that is measurable, documented in the EMR and updated regularly; develop visit order frequencies for follow‑up visits to provide spiritual support. - Hospice chaplains ONLY:
Provide spiritual health care in the home hospice, IPU (inpatient unit), and GIP (general inpatient) and ED (emergency department)/outpatient settings as assigned. - Outpatient chaplains ONLY:
Provide spiritual health care in Well Star Medical Group settings, including attending to urgent spiritual needs in other Well Star outpatient settings.
