Job Overview

Under general supervision, provide Nursing Care Coordination in a specialized Public Health program that serves Children and Youth with Special Medical Needs, per CMS policy and procedure manual.

Job Responsibilities
  • Under supervision, provides professional nursing care in accordance with established nurse protocols, policies, and/or procedures.
  • Conducts initial, annual, and other periodic assessments including documenting medical and social needs and events of clients with special needs and their families via Web Ex, at clinic, or in the home within designated counties.
  • Partners with the family and caregivers to assess a client’s condition and develop a plan of care.
  • Learns about and interacts with community partners, other public and private service agencies, and local/regional resources.
  • Serves as an advocate and helps overcome obstacles in order to connect clients with chronic illnesses to appropriate specialty care and community supports.
  • Educates clients and caregivers about the need to transition to adult special services.
  • Takes notes and provides assistance in our CMS Orthopedic Clinic as needed.
  • Records patient care including all pertinent observations and information in client chart and EMR.
  • Learns how to take a referral, determines medical and financial eligibility for the CMS program, and refers to other PH programs.
  • Understands and documents appropriately for billing and for program reports.
  • Incorporates strategies for interacting with persons from diverse backgrounds e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, sexual orientation, professional, religious affiliation, mental and physical capabilities.
  • Prepares to respond to public health emergencies and respond as directed within your scope of practice.
  • Uses individual, team, and organizational learning opportunities for personal and professional development.
Travel Requirements

This position requires travel to all 12 District 4 Counties. Travel reimbursement will be based on current State of Georgia travel regulations.

Minimum Qualifications
  • PH Registered Nurse Level 1
    Annual Salary - $51,811.56

    Associate’s degree in Nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
    Note:
    No experience required

  • PH Registered Nurse Level 2
    Annual Salary - $58,547.06

    Bachelor’s degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree in Nursing from an accredited college or university AND one year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Nurse license AND CPR certification.

    Note:
    Level of RN position hired will be based on number of years performing nursing duties consistent with Public Health Nursing Experience.
