• Perform Social Service assessment and address counseling needs.
• Maintain thorough knowledge of referral resources throughout the service area.
• Facilitate home health and nursing home placements and transfers.
• Provide positive customer relations with patients, families, physicians, department employees, other hospital departments, referral resources, service agencies, and others.
• Practice proactive discharge planning and coordination of care.
• Maintain accurate and timely records, including clinical and progress notes, according to department policy.

• Master’s in Social Work with Case Management certification preferred.
• One (1) year of social work experience in a health care setting.
• Computer skills at data entry level.
• Must maintain required hours of in‑service per year to meet CEU requirements (20 hours) as stipulated by the Louisiana State Board of Social Work Examiners.
• Ability to communicate effectively with patients, families, peers, medical personnel, and community referral agencies.

• Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time.
• Must be able to respond quickly and effectively to emergency and non‑emergent situations.
• May be required to assist in controlling disorderly conduct or combative patients.
• Must be able to exchange accurate information with patient, family, peers, and medical personnel.
