Licensed Practical Nurse LPN; Hospice - Quality Assurance

We are currently seeking qualified candidates for a full time Quality Assurance (QA) LPN Nurse position in the Lake Charles area.

A Quality Assurance LPN is responsible for assuring quality control of all health services provided by the Agency and for reviewing documentation for compliance and appropriateness of care.

• Must have an associate degree (A.A.), or equivalent from a two (2) year college.
• Must have at least one year’s experience in hospice, home health or a related field.
• Must have a current State LPN or LVN license.
• Must be able to speak and write effectively.
• Must work well with other disciplines to coordinate patient care.
• Must be able to prepare forms for pre and post payment audits.
• Must have a current State driver’s license.
• Experience in reviewing nurses’ notes for completeness, compliance with physicians’ orders, authorized services, regulations required.
• Experience in reviewing admission packets required.
• Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, C.N.A. care plan.
• Conducts one-on-one in-services for documentation problems as needed.
• Effective written and verbal communication.
• Ability to travel - must have reliable transportation.
• Clean background and drug screen.
