- As an RN Case Manager, youll be a vital member of the team, providing hands-on skilled nursing care and overseeing the delivery of services to clients within their homes.
- Collaborate with clients, families, and healthcare teams to develop and implement goal-centered care plans, considering both quality and financial aspects of treatment.
- Provide hands-on skilled nursing care to clients of varying ages and acuity levels.
- Coordinate care with families, caregivers, and other healthcare providers.
- Medication Management:
Ensure proper medication administration and reconciliation. - Client Monitoring:
Regularly monitor clients conditions and report any changes to the appropriate personnel. - Provide education and support to clients, families, and caregivers.
- Ensure accurate and timely documentation of client care and progress.
- Ensure the safety needs of clients are addressed through home environment assessments and individualized abuse prevention plans.
- Provide clinical oversight, delegation, and leadership to other licensed and unlicensed personnel.
- Documentation:
Maintain accurate and up-to-date client records. - Compliance:
Adhere to all relevant regulations, including OSHA and HIPAA. - Manage your schedule effectively and participate in the on-call phone rotation on occasion.
- Currently licensed and unrestricted as a Registered Nurse (RN) in the State of Minnesota and Wisconsin.
- Certified in CPR and have a negative TB skin test or chest x-ray.
- Minimum of one (1) years of experience in a health care facility or home care setting. Home Health
Experience:
Two or more years of clinical experience, with one year in a home health agency, primary care clinic, or health facility, is preferred. - Driving: A valid driver s license and reliable transportation are necessary.
- Demonstrates critical thinking skills and the ability to make appropriate clinical judgments.
- Excellent written and oral communication skills.
- Proficiency in using computer systems and electronic charting is essential.
