The Clinical Pharmacist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and regulatory and accreditation standards. The Clinical Pharmacist is responsible for identifying, preventing, and/or resolving medication‑related problems, providing authoritative information to other health care professionals, patients and the public, and ensuring that the informational, clinical, and drug distribution components of pharmacy services function as an integrated system to provide patient‑centered care.
Responsibilities- Interpret medication orders to ensure safety and proper use of medications and alert to compatibilities, incompatibilities, medication duplication, allergies, and potential administration errors. Coordinate the resolution of medication‑related problems between pharmacy, nursing, and prescribers.
- Monitor medication therapy for significant drug‑drug, drug‑diet, or drug‑laboratory test interactions; intervene in circumstances of significant interactions, therapeutic duplications, and other situations requiring pharmacodynamic and pharmacokinetic expertise.
- Evaluate and document that selected medication(s) achieve desired therapeutic outcomes; communicate potential problems and likely solutions to the prescriber.
- Act as a consultant on pharmacokinetics and adverse reactions to physicians, nurses, and other health care professionals; participate in therapeutic drug monitoring or intervention programs, adjusting therapy based on patient condition and laboratory results.
- Process all medication order information into the Electronic Health Record to ensure accurate records of dispensing and charging.
- Maintain a comprehensive pharmacy patient profile, including age, weight, allergies, diagnosis, estimated and actual pharmacokinetic parameters, and other pertinent information.
- Oversee packaging, labeling, and distribution of medications; monitor scheduled medication usage on nursing units and maintain controlled inventories.
- Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition.
- Verify medication orders before delivery to patient care areas and resolve discrepancies as necessary.
- Review laboratory data for medication appropriateness and recommend changes when needed.
- Develop and present orientation, education, and training programs for medical, nursing, hospital and pharmacy staff; assist in educating pharmacy students, residents, and fellows.
- Educate patients and their families about specific medications.
- Respond to emergency situations in medication management, e.g., cardiac arrest response.
- Provide accurate and timely medication information and teaching to health professionals or patients.
- Maintain the hospital formulary and limit medication usage to formulary items when appropriate.
- Document all clinical activities and interventions performed; collect data and maintain logs and records as required.
- Communicate professional knowledge effectively orally and in writing to patients and other health professionals.
- Participate in quality improvement and medication use review activities.
- Organize, prioritize, and plan workload; collaborate with fellow pharmacists; supervise technicians and ensure department objectives are accomplished.
- Review restocking of automated dispensing cabinets (ADCs) and resolve problems.
