Job Description- 72 hours per pay period
- Evening shift: 3:00 pm – 11:30 pm
- Schedule:
7 days on / 7 days off
- Interpret medication orders to ensure safety and proper use, being alert to compatibilities, incompatibilities, medication duplications, allergies, and potential medication administration errors. Coordinate the resolution of medication‑related problems with pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.
- Monitor medication therapy for significant drug‑drug, drug‑diet, or drug‑laboratory test interactions. Intervene in cases of significant interactions, therapeutic duplications, and situations where pharmacodynamics and pharmacokinetic expertise is warranted.
- Evaluate and document that the medication(s) selected are appropriate for achieving the desired therapeutic outcome. Communicate potential problems and likely solutions to the prescriber.
- Act as a consultant on pharmacokinetics and adverse reactions to physicians, nurses, and other health‑care professionals. Participate in therapeutic drug monitoring or intervention programs, adjusting therapy as indicated by patient condition and laboratory results.
- Process all medication order information into the Electronic Health Record to ensure accuracy of records regarding past, current, and future dispensing and charging.
- Maintain a comprehensive pharmacy patient profile, including age, maturation, weight, allergies, diagnosis, estimated and actual pharmacokinetic parameters, and pertinent information that may affect medication therapy decisions.
- Oversee the packaging, labeling, and distribution of medications for all patients and employees according to established policies, procedures, and guidelines. Monitor the use of scheduled medications on nursing units and maintain controlled inventories in dispensing areas.
- Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
- Verify medication orders before delivery to patient care areas, ensuring accuracy and completeness. Resolve discrepancies as necessary.
- Review all laboratory data for appropriateness of medication use and recommend changes when needed, including microbial culture and sensitivity data, toxicology, and therapeutic drug monitoring.
- Develop and present orientation, education, and training programs for medical, nursing, hospital, and pharmacy staff, and assist in educating pharmacy students, residents, and fellows.
- Educate patients and their families about specific medications.
- Respond to emergency situations to assist with medication management (e.g., response to cardiac arrests).
- Provide accurate and timely medication information and/or teaching to health‑care professionals or patients.
- Maintain the hospital formulary and act to limit medication usage to formulary items when appropriate.
- Document all clinical activities and interventions performed. Collect data, maintain logs, records, and other documentation as required by the department.
- Communicate professional knowledge effectively in both oral and written forms to patients and other health professionals.
- Participate in quality improvement and medication use review activities of the department.
- Organize, prioritize, and plan workload within given time constraints. Work cooperatively with fellow pharmacists to oversee and direct the activities of the work area, assigning work to technicians, and ensuring that established department objectives are achieved.
- Review restocking of automated dispensing cabinets (ADCs) throughout the hospital and resolve any problems.
