Physical Therapist - Outpatient (Full Time Days)

• Outpatient clinic hours:
  Monday–Thursday 7 am–7 pm, Friday 7 am–5 pm.
• Schedule:
  
  40 hours/week, primarily daytime hours, two evening shifts per week.

• Licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by the State of Illinois.
• The position includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement.
• Serves as a role model for all therapists and clinical interns in professional, clinical, communication, education, and leadership activities.

• Provide accurate information and timely updates to patients and customers; address questions and concerns promptly or identify appropriate resources.

• Demonstrate essential skill competencies in selecting appropriate evaluation, use of treatment modalities, equipment and protocols; perform re‑evaluations and adjust the treatment plan as required.
• Assume complete responsibility for the course of patient management from initial contact to discharge for functional progress, safety and treatment outcome, evidenced by documentation.
• Adhere to department policy regarding evaluations, progress notes, and discharge summaries; document all notes in the EMR.
• Complete daily notes that reflect patient’s response to treatment and assessment of status, meeting all documentation requirements.
• Provide educational materials and methods appropriate to the patient’s level of understanding and motivation.
• Participate in committees, department processes, and special projects as per clinical expertise and performance plan.
• Work with healthcare teams (physician, nurse, case manager) and participate in internal and external marketing per department expectations.
• Create a culture of respectful communication, listening, and timely acknowledgement of contributions.
• Accept constructive feedback and make timely changes as evidenced by coaching forms and team feedback.
• Proactively complete non‑patient‑care responsibilities within established timelines (coverage notes, PTO/schedule change requests, bonus pay forms).
• Contribute to educational efforts of department/clinic (in‑services, journal clubs, new‑hire orientation).
