This position provides skilled nursing care to patients in their homes throughout Lake Forest and surrounding areas. The role focuses on delivering compassionate, patient-centered care while maintaining flexibility and autonomy. The nurse will manage visits, documentation, and care coordination within a supportive, team-based environment.
Key responsibilities- Deliver professional nursing care to patients in home settings
- Assess, plan, and implement individualized care plans
- Complete documentation and charting accurately and on time
- Collaborate with care teams to ensure quality outcomes
- Manage time and patient schedules independently
- Monday through Friday, flexible scheduling
- Minimal weekend coverage required
- Pay-per-visit structure
- $10,000 bonus paid quarterly
- Local travel required; mileage reimbursement provided
Required
- Associate degree or higher in Nursing
- At least 1 year of experience as an RN (Home Health experience preferred)
- Active Registered Nurse (RN) license in Illinois
- Current BLS and CPR certifications (support available if not current)
- Valid driver’s license, reliable transportation, and current auto insurance
