Registered Nurse – Alternate Care Space (Full Time, Nights) – Sign On Bonus Eligible

Salary range: $34.50–$55.20 hourly. Placement is dependent on experience, internal equity, and collective bargaining agreements for unionized positions.

Internal employees and rehires who left Northwestern Medicine within one year are not eligible for the sign‑on bonus, except for new graduate RNs who are seeking their first nursing position.
Responsibilities- Perform comprehensive assessment on all assigned patients and document findings.
- Communicate changes in patient condition promptly.
- Use nursing diagnosis and assessment data to set measurable goals on the plan of care; modify plan as needed.
- Identify and prioritize patient needs and/or nursing diagnoses and document them.
- Collaborate with patient, family, and other health‑care team members when planning care and document accordingly.
- Assess patients for discharge planning and make appropriate referrals, including palliative care.
- Evaluate and document patient response to interventions and adjust plan of care.
- Provide discharge instructions, medication reconciliation, and educate patients/families on alerts, medications and use of the patient portal.
- Support patient/family/spiritual/cultural needs during teaching sessions; determine learning level and readiness; document education record.
- Utilize teach‑back methodology to educate patients about medication side effects and new medications.
- Distribute Krames for patient education and utilize Culture Vision as a resource in care planning.
- Treat patients, families, and staff in a caring manner with respect and dignity.
- Acknowledge and value contributions of patient’s significant others as part of the plan of care.
- Adhere to ethical standards and call for an ethics consult when appropriate.
- Implement interventions based on evidence‑based care and maintain evidence knowledge using resources such as Ovid/Joanna Briggs, Up To Date , Culture Vision.
- Demonstrate active involvement in clinical ladder projects or process improvement at unit/organization level.
- Ensure safe medication administration with bar‑coding and promote patient safety.
- Communicate complete, timely, and accurate information during bedside shift reports and hand‑offs.
- Round hourly with patient‑care technicians.
- Participate in unit safety initiatives and quality metrics.
- Prevent pressure ulcers, falls, CAUTIs, and central line infections when applicable.
- Participate in unit huddles and serve as a resource and role model for new staff, students, and system personnel.
- Assist with preceptor, unit orientation, and mentoring as requested.
- Provide feedback to colleagues and engage in peer evaluations.
- Maintain appropriate level of care orders in EMR for all admissions and understand supply charging.
- Participate in value‑based initiatives and reduce overtime hours.
- Ensure complete electronic and paper documentation; use guardrails for Alaris pump and bed‑management system.
- Identify new technologies for unit, hospital, or system use and engage in CEOC activities.
- Attend internal professional development offerings and obtain specialty certification.
- Demonstrate accountability for professional competencies, learning, dress code, and tolerance.
- Participate fully in professional organizations.
Required
- Bachelor of Science in Nursing (BSN)
- Current State of Illinois Registered Professional Nurse
- Basic Life Support

- National certification in area of specialty
