Job DescriptionThe Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of department leadership, uniformly reflects the mission, vision, and values, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies and accreditation standards.
Relationship to Patient/Family and Community Care Coordination- Performs a comprehensive assessment on all assigned patients.
- Communicates changes in patient’s condition promptly.
- Utilizes appropriate nursing diagnosis to derive realistic outcomes and documents measurable goals on the plan of care.
- Identifies and prioritizes patient needs and nursing diagnoses.
- Collaborates with patient, family, and other team members when planning patient care.
- Assesses patients for discharge planning and makes appropriate referrals.
- Evaluates and documents patient response to nursing interventions and adjusts the plan of care as necessary.
- Provides completed discharge instructions, medication reconciliation, and information about the patient portal.
- Supports patient/family/spiritual/cultural needs and documents education records.
- Utilizes teach‑back methodology for medication education.
- Uses Krames for patient education on clinical conditions.
- Applies Culture Vision as a resource in care planning.
- Treats patients, families, and staff with respect and dignity.
- Values contributions of patient’s significant others in care planning.
- Adheres to ethical standards of practice.
- Calls for an ethics consult when appropriate.
- Implements interventions consistent with evidence‑based care.
- Utilizes resources such as Ovid/Joanna Briggs, Up‑to‑Date, Culture Vision.
- Participates in clinical LADDER projects or process improvement initiatives.
- Demonstrates safe medication administration with bar coding.
- Promotes the health, safety, and rights of patients.
- Communicates complete, timely information at bedside shift reports.
- Rounds hourly in collaboration with patient‑care technicians.
- Maintains a safe environment of excellence.
- Participates in quality initiatives and unit safety facilitation.
- Engages in unit huddles.
- Oversees and directs patient‑care or support technicians.
- Demonstrates interpersonal skills while addressing conflict.
- Performs charge nurse/preceptor duties when assigned.
- Leads when patient’s condition changes and coordinates with physicians.
- Serves as a resource and role model for new staff and students.
- Precepts, orients, and mentors as requested.
- Provides peer evaluation feedback.
- Collaborates with hospital services.
- Identifies barriers to effective clinical practice and communicates with leadership.
- Ensures appropriate level of care is documented for all admissions.
- Adheres to supply charging and participates in value‑based initiatives.
- Commits to reducing utilization overtime hours.
- Ensures complete electronic and paper documentation.
- Utilizes guardrails for Alaris pump, physiological monitoring, bed management system, and identifies new technologies.
- Participates in CEOC activities for medical record optimization.
- Attends internal professional development offerings such as Schwartz Rounds and Nursing Grand Rounds.
- Obtains specialty certification.
- Maintains professional practice competencies and compliance with dress code.
- Participates in professional organizations.
- Takes assigned lunch breaks.
