Salary range: $34.50 - $55.20 per hour. Placement within the range depends on experience, internal equity, labor union guidelines and collective bargaining agreements.

DescriptionThe Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of the department leadership, the RN consistently reflects the mission, vision and values of NM, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and regulatory and accreditation standards.
ResponsibilitiesRelationship to Patient/Family and Community Care Coordination
- Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation.
- Communicates changes in patient’s condition promptly.
- Uses nursing diagnosis and assessment data to derive realistic outcomes and documents measurable goals on the plan of care; modifies care plan when indicated.
- Identifies and prioritizes patient needs and nursing diagnoses and documents them.
- Collaborates with patient, family and other members of the health care team when planning patient care and documents accordingly.
- Assesses patients for discharge planning and makes appropriate referrals as necessary.
- Evaluates and documents the patient response to nursing interventions and adjusts the plan of care as necessary.
- Ensures completed discharge instructions, medication reconciliation and provides information to patient/family regarding patient portal and other resources.
- Supports patient, family, spiritual and cultural needs when assessing the patient and the family for teaching; identifies learning level and readiness, and documents education record.
- Utilizes teach‑back methodology for medication education.
- Distributes Krames patient education materials.
- Utilizes Culture Vision as a resource in care planning and delivery.
- Treats patients, families and staff with respect, dignity and caring manner.
- Values contributions of patient’s significant others as integral part of the plan of care.
- Adheres to ethical standards of practice.
- Calls for an ethics consult when appropriate.
- Implements nursing interventions consistent with the established plan of care based on evidence‑based practice.
- Supports professional image and science of nursing through use of best available evidence from Ovid/Joanna Briggs, Up to Date, Culture Vision and other reliable sources.
- Participates in clinical LADDER project or process improvement at unit or organization level.
- Demonstrates safe medication administration via bar coding.
- Promotes and protects patient health, safety and rights.
- Conveys complete, timely, and accurate information during bedside shift report/hand‑off.
- Rounds hourly with patient care technician.
- Maintains a safe environment of excellence.
- Participates in unit‑based quality metrics and exceptional care dashboard.
- Prevents pressure ulcers, falls, CAUTI’s and central line infections.
- Acts as unit safety facilitator or quality initiative resource as requested.
- Participates in unit huddles.
- Oversees and directs work of patient care or support techs; maintains accountability for assigned patients.
- Demonstrates effective interpersonal skills in leading and directing others; deals with conflict directly.
- Proficient in charge nurse/preceptor role when assigned; coordinates patient care for entire unit.
