Social Worker

Geneva Lake Manor invites applicants for a full‑time Social Worker responsible for promoting the emotional and psychosocial needs of residents in a long‑term care setting.

Base Pay Range

$24.00/hr – $25.00/hr

Key Responsibilities
  • Conduct resident interviews, assess psychosocial factors, and develop individualized care plans following state and federal regulations.
  • Coordinate multidisciplinary care planning meetings and discharge planning with community resources.
  • Maintain confidential resident information and provide annual in‑service training on resident rights and confidentiality.
  • Assist the financial manager in explaining Medicaid/Medicare and other financial programs to residents and families.
  • Provide consultation to staff and community agencies to support resident needs and family relationships.
  • Develop and maintain departmental policies, procedures, and educational programs for residents, families, and the community.
Qualifications
  • Bachelor’s degree in Social Work or Human Services.
  • Minimum one year of experience as a Social Worker in a long‑term care or rehabilitation setting.
  • Knowledge of community resources and ability to secure them for residents.
  • Strong organizational, oral, and written communication skills.
  • Fluency in English for reading, writing, and speaking.
  • Commitment to a multidisciplinary approach and adherence to state and federal regulations.
  • Flexibility to work varied schedules as needed.
