Summary

Responsible for the planning, provision, and coordination of all aspects of direct patient care in accordance with established clinic and departmental policies. Coordinate with other patient‑team personnel to maintain professional standards and use independent judgment when supervising PTAs and Rehab Techs/Aides.

Essential Duties and Responsibilities
  • Provide high‑quality patient care and assessment in accordance with clinic policies.
  • Perform patient evaluation, summarize findings, and develop individualized plan of care.
  • Implement treatment per physician orders and plan of care.
  • Evaluate effectiveness of treatment and revise plan as indicated.
  • Collaborate with patient, family, caregivers, and healthcare team to promote maximum benefit of care.
  • Communicate with referral sources such as physicians, NCM, ADJ, etc.
  • Assess educational needs of patients and team to promote maximum benefit.
  • Apply appropriate clinical techniques and procedures.
  • Plan and execute appropriate discharge plans.
  • Assess safety risks and implement precautions.
  • Comply with security and safety standards.
  • Utilize leadership skills in delegating, organizing, and educating staff.
  • Participate in quality improvement process.
  • Achieve efficiency standards such as skilled units per visit, visits per day, and evaluations plus two‑skilled units.
  • Complete timely, competent, and compliant documentation using Agile

    RPM.
  • Travel to other local clinics as assigned.
Qualifications
  • Master’s or Doctorate degree in Physical Therapy.
  • Current licensed Physical Therapist in the state of employment.
  • State registration and/or certification as per state regulations.
Physical Demands
  • Frequency:
    Stand, walk, sit, reach, climb, balance, stoop, kneel, crouch or crawl regularly.
  • Lift/move up to 25 pounds regularly and occasionally more than 100 pounds.
  • Reasonable accommodations available for employees with disabilities.
