North Lake Care Center, a premier provider of skilled nursing services, is seeking a per diem Physical Therapist (PT) to join our team in Lake Park, Florida. As a Physical Therapist (PT) with our organization, you will play a vital role in helping our patients achieve their maximum potential and live their lives to the fullest. About the Role:- Evaluating and treating patients with a wide range of diagnoses and conditions
- Developing and implementing individualized treatment plans to achieve optimal patient outcomes
- Providing hands-on therapy treatment to patients, including exercises, stretches, and other interventions
- Conducting patient education sessions to promote independence and self-care
- Collaborating with other healthcare professionals to ensure seamless patient care
- Maintaining accurate and up-to-date patient records and reports
- Bachelor's or Master's degree in Physical Therapy from an accredited program
- Current licensure as a Physical Therapist in the state of Florida
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Familiarity with electronic medical records (EMRs)
