Responsibilities- Provide skilled nursing care to residents under the supervision of a delegation RN or LPN
- Conduct resident assessments, develop care plans, and implement interventions to meet individual needs
- Administer medications, vaccinations, and treatments as prescribed by physicians
- Monitor and report changes in resident condition to the healthcare team
- Collaborate with other healthcare professionals to develop and implement comprehensive care plans
- Maintain accurate and timely records of resident care and services
- Participate in quality improvement initiatives to improve care and outcomes
