Salary/Hourly Rate: $44,850 annually
Reports To: Executive Director/ Senior Director of Programs

Position Status: Full Time W-2
Position OverviewThe Rehabilitation Counselor (RC) supports individuals diagnosed with severe and persistent mental illness in regaining and developing the skills necessary for independent living, self-care, community engagement, and life goal achievement. Working closely with Rehabilitation Specialists (RS), the RC ensures participants receive consistent, person-centered psychiatric rehabilitation services, both individually and in group settings, in accordance with COMAR regulations and Lead4

- Assist PRP staff and Program Director in implementing individual rehabilitation plans (IRPs), goals, and service delivery strategies.
- Ensure quality and compliance across documentation, service frequency, and regulatory standards.
- Support coordination of services across all PRP sites and travel as needed to ensure consistency and participant engagement.
- Provide daily oversight and support to clients in areas such as Social skills development, Interpersonal communication, Daily living activities (ADLs), Coping mechanisms, Behavioral health goal setting, and Community reintegration.
- Participate in treatment planning and implement interventions designed by the clinical team.
- Collaborate closely with therapists, psychiatric providers, guardians, and relevant community stakeholders.
- Lead or co-facilitate group sessions on wellness, communication, behavior management, vocational readiness, and more.
- Offer individual coaching and case management around housing, employment, education, and other recovery-oriented goals.
- Complete and submit timely progress notes, visit documentation, and service logs using approved formats and platforms.
- Ensure monthly documentation is completed by COB the day before the last day of the month.
- Update and submit Treatment Plans every 90 days in accordance with COMAR standards.
- Maintain all required signatures and recordkeeping in participant files.
- Attend and participate in monthly staff meetings, team trainings, and individual supervision sessions.
- Provide feedback to the treatment team and participate in care planning, especially around discharge and aftercare.
- Actively contribute to PRP program development, outreach, and internal quality improvement.
- Participate in community engagement and public education efforts related to PRP services and behavioral health advocacy.
- Foster client autonomy, self-determination, and motivation through strength-based practices.
- Provide crisis de-escalation and emotional support as needed.
- Maintain a professional, ethical, and trauma-informed approach in all interactions.
- Build and maintain relationships with community partners, such as therapists, hospitals, behavioral health agencies, schools, and courts.
- Ensure documentation and service delivery adhere to all state and federal regulations, including Medicaid billing and CARF standards.
Required
- High School Diploma or GED
- Experience working with individuals with mental illness or behavioral health needs
Preferred
- Bachelor’s Degree (B.A./B.S.) in Social Work, Psychology, Human Services, or a related field
- Knowledge of COMAR standards and psychiatric rehabilitation services
- Experience with electronic health record systems and documentation
- Strong interpersonal and communication skills
- Problem-solving and critical thinking
- Professional, empathetic, and client-centered approach
- Familiarity with trauma-informed care and recovery models
- Ability to work independently and as part of a multidisciplinary team
- Reliable transportation and willingness to travel across service areas
The position requires regular travel throughout the state to client homes, community sites, and office locations. Must be able to sit, stand, walk, and use electronic devices for extended periods.
