Job Title: Returning Citizens Initiative (RCI) Case Manager

Reports To: Executive Director/ Senior Director of Programs

Salary/Hourly Rate: $46,800 annually

Direct Reports: Program Coordinator

Position Status: Full Time W-2

Location: Lower Eastern Shore, Maryland

Position Overview

The RCI (Returning Citizens Initiative) Case Manager provides intensive, personalized support to individuals reentering the community after incarceration. This position focuses on intake and assessment of new referrals, goal setting, and addressing barriers that hinder successful reintegration. Through direct mentorship, resource navigation, and collaborative planning, the Case Manager helps returning citizens become productive, self-sufficient members of society. This role is primarily in person, requiring strong engagement and presence in the community.

Key Responsibilities

Intake, Assessment and Case Planning

  • Facilitate the intake process for new participants, ensuring accurate assessment of needs, eligibility, and risk factors.
  • Develop individualized case plans focused on short- and long-term goals related to employment, housing, education, and community reintegration.
  • Continuously monitor participant progress, adjust plans as needed, and support clients in addressing barriers as they arise.
Direct Support and Mentorship
  • Provide one-on-one coaching and peer mentorship to encourage personal accountability, resilience, and successful goal attainment.
  • Assist clients in completing job applications, building resumes, and preparing for interviews.
  • Support participants with job searches, housing applications, education or vocational program enrollment, and other community-based opportunities.
Community Engagement and Outreach
  • Collaborate with parole/probation officers, service providers, and community organizations to build a strong support network for each client.
  • Help coordinate transportation, appointments, and group support sessions to facilitate consistent engagement.
Documentation & Compliance
  • Maintain up-to-date, detailed case notes and service documentation in case management systems.
  • Track all referrals, participant interactions, goal progress, and outcomes to support compliance and performance reporting.
  • Participate in audits, program evaluations, and regular quality assurance checks as needed.
Program Support & Team Collaboration
  • Attend and contribute to regular team meetings, supervision sessions, and professional development training.
  • Support program outreach and visibility by attending community events and stakeholder meetings.
  • Act as a representative of Lead4

    Life’s mission and values in all interactions with partners and participants.

Minimum Requirements

  • Associate or bachelor’s degree in social work, Human Services, Criminal Justice, Psychology, or related field (OR equivalent experience).
  • Minimum of 2 years of experience working with justice-involved individuals or in community-based case management.
  • Lived experience with reentry and successful reintegration is highly valued.
  • Strong knowledge of community resources related to employment, housing, healthcare, education, and behavioral health.
  • Excellent communication, organization, and problem-solving skills.
  • Must possess a valid driver’s license and have reliable transportation for community-based work.

Desired Qualifications

  • Prior experience facilitating motivational or peer support groups within correctional or reentry settings.
  • Familiarity with reentry challenges, trauma-informed care, and culturally responsive practices.
  • Experience using electronic case management systems or databases.
  • Ability to de-escalate conflict, build trust, and maintain strong professional boundaries.
  • Knowledge of Maryland’s reentry and probation/parole systems.
