Job DescriptionThe Registered Nurse, RN, is a member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN is responsible for coordinating care, continuously assessing each patient's and family's needs, and implementing the interdisciplinary plan of care.
Primary Responsibilities- Current nursing license in the state of practice.
- Minimum of two years of nursing experience in hospice, home health, or community health within the last five years.
- Reliable transportation and a valid driver's license.
- Available telephone contact.
Regularly required to lift and transfer patients, stand or walk for extended periods, and communicate effectively. Vision and mobility requirements are specified to ensure safety and quality of care.
Qualifications- Licensed to practice nursing in the state where employed.
- Experience in hospice or related fields preferred.
