• Location: Lake Worth, Florida
• Shift: Full-Time | Day Shift, 9:00 AM - 5:00 PM
• Pay Rate: $42-$46 per hour (based on experience)

Palm Beach Nursing Center is seeking a skilled and detail‑oriented RN MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process. The ideal candidate will be well‑versed in PDPM and Medicare documentation, ensuring compliance with state and federal regulations while supporting exceptional resident care.

• Accurately complete and coordinate all MDS assessments in compliance with regulations.
• Develop and maintain individualized care plans based on assessment outcomes.
• Collaborate with interdisciplinary teams to ensure continuity and quality of care.
• Monitor PDPM requirements and ensure documentation supports Medicare claims.
• Review and analyze QI/QM reports, implementing corrective actions when necessary.
• Participate in surveys, audits, and inspections.
• Submit assessments electronically and maintain organized records and schedules.
• Educate staff on MDS/PDPM updates and best practices.

• Active RN license in the state of Florida.
• Proven experience with PDPM and Medicare documentation.
• Minimum of 2 years’ experience in a skilled nursing facility preferred.
• Strong knowledge of MDS 3.0, care planning, and RAI processes.
• Excellent communication, leadership, and organizational skills.
