TEEMA is seeking an experienced Registered Nurse (RN) Care Manager to support home health initiatives across clinics, centers, and patients’ homes. You will assess needs, build and implement care plans, and coordinate services to optimize patient outcomes.
The role requires an active RN license, BSN preferred, and engagement in a 24/7 on-call rotation. Travel to patient homes and on-site work are expected as part of care delivery.
