Empath Hospice, a part of Empath Health, is seeking a Social Worker to support patients and families during their end‑of‑life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve in Polk, Hardee, and Highlands counties.

• Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
• Provide individual and family counseling, crisis intervention, and bereavement support.
• Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
• Assist with financial resources, Medicare/Medicaid, and long‑term care planning.
• Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
• Support patients and families with advance directives and memorial planning.
• Educate the community on hospice care.

• Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).
• Licensure: Must hold one of the following Florida licenses or be license‑eligible (Registered Intern) within 90 days of hire:
  • Social Work (LCSW) or Social Work (RCSWI)
  • Mental Health Counselor (LMHC) or Mental Health Counselor (RMHCI)
  • Marriage and Family Therapist (LMFT) or Marriage and Family Therapist (RMFTI)
• For field‑based positions, employees must have reliable transportation and provide proof of valid automobile insurance.
