Employees in this position perform a wide variety of duties related to the care, treatment, and rehabilitation of clients. These employees are aware of available community resources and function as a member of a treatment team.

- Receives contact from outside resources for referral into the Tri-County outpatient system of care. Under direction from supervision, the incumbent will:
- Initiate contact with the parent(s) to begin the engagement and enrollment process within three (3) business days of receiving a referral.
- If contact with the family is successful, the incumbent will schedule next available appointment and notify referral source of appointment date/time. The incumbent shall prioritize all child welfare referrals.
- The Provider Receipt portion of the Child Welfare Referral Form (Exhibit
A) will be completed by the incumbent and returned via email to the referral source. - If contact with the family is unsuccessful after three (3) attempts, the incumbent will notify the referral source within two (2) business days of the last failed contact. If the referral source doesn’t respond within two (2) business days, the incumbent will notify DCF/HFC point of contact via email.
- If it is determined the referral should be closed, the Provider Receipt portion of the Child Welfare Referral Form (Exhibit
A) will be completed by the incumbent and returned via email to the referral source. - Each month, the incumbent will report how many Child Welfare Referral Forms were received, how many engaged in services, and outcomes on those services.
- Under the supervision of the supervisor the employee records individual case information, updates case files, and provides case status information in order to provide a complete record of services.
- Confers with supervisor and other team members at staff meetings, providing and/or receiving information as necessary to assure proper case management.
- Performs other reasonable and related duties as assigned.
- Requires a high school diploma or G.E.D. and, preferably, one year of experience in a mental health or addictions treatment setting.
- Requires knowledge of addictions treatment and/or mental health treatment program and supportive services.
- Requires general understanding of the causes, nature, and treatment of substance abuse and/or mental health programs.
