CAT Team Case Manager

This member of the multidisciplinary team has the responsibility to carry out rehabilitation and support functions; assist in treatment, substance misuse services, education, support, coordination of care, linking, assessing, monitoring, planning, and consultation to families; and crisis intervention under the clinical supervision of the CAT Team Program Manager. This position will be based in Polk County but will be assigned clients residing throughout the Tri-County service area of Polk, Highlands, and Hardee Counties.

• Attends all scheduled work hours, meeting, training, and other center functions
• Complete monthly home visits with clients as directed by CAT Team 2 Program Manager
• Complete monthly school visits to coordinate with school staff to strengthen community support system for client and to obtain needed school collateral and records
• Provide case management for an assigned group of people including coordinating and monitoring the activities of the individual treatment team; provide the individual with the support necessary for symptom management; ensure immediate changes are made in treatment plans as persons’ needs change; educate and support persons’ families; and advocate for persons’ rights and preferences
• Complete all daily progress note in Credible and assessments in a timely manner as mandated in the CAT Team policies and procedures
• Complete required assessments for all assigned clients within specified time frames as directed by CAT Team Program Manager
• Consult with community agencies and families to maintain coordination in the treatment process
• Document client’s progress to maintain a permanent record of client activity according to established methods and procedures
• Participate in daily staff organizational meetings and treatment planning review meetings
• Participate in providing substance use and treatment support services, including coordination with any needed referrals
• Assist in linking client transportation for scheduled medical, dental, or other appointments as needed and determined to be in the best therapeutic interest of the participant as assigned (e.g., arrange for cabs, access bus line, apply for Handibus, etc.).
• Assist and support CAT Team participants to carry out personal hygiene and grooming tasks
• Assist CAT Team participants’ families to find and maintain a safe and affordable place to live--apartment hunting, landlord negotiations, and procuring necessities to set up housekeeping
• Organize and lead individual and group social, educational, and recreational activities to structure CAT Team participants' time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support
• Provide practical help and supports, advocacy, coordination, side‑by‑side individualized support, problem solving, direct assistance, training, and supervision to help CAT Team participants obtain the necessities of daily living including medical and dental health care; legal and advocacy services; financial support; money‑management services; transportation; social‑skill development; and assertiveness training to increase their social and interpersonal activities in community settings
