Hours Day HCN accompanying client to school

Days Sundays through Wednesdays & Every other Friday 9 or 10 hour shifts

Overnights Every Monday & Wednesday & Every other Friday & Saturday 10 hour shifts

Hours Days Monday through Thursdays 10 hour shifts

Client Specifics 15 year old with trach vent and a g tube

What Youll Do Provide skilled nursing care in home and school settings Assess and monitor your patients condition and communicate changes with the care team Administer medications treatments and prescribed therapies Provide trach ventilator airway and G tube care as needed Maintain accurate documentation and support families through education Follow individualized care plans and maintain a safe care environment
