Key Responsibilities
- Oversee and manage the day‑to‑day operations of the social services department, ensuring high standards of care and service delivery.
- Develop, implement, and evaluate social service programs to meet the psychosocial needs of residents and their families.
- Provide counseling, support, and advocacy for residents facing adjustment, emotional, or discharge planning challenges.
- Lead and participate in interdisciplinary care planning meetings, ensuring that social service goals are integrated into comprehensive resident care plans.
- Facilitate resident admissions, transfers, and discharges, coordinating with residents, families, and external agencies.
- Ensure the department's compliance with all federal, state, and local regulations pertaining to social services in long‑term care.
- Supervise, mentor, and evaluate social work staff, fostering a collaborative and high‑performing team.
- Develop and maintain strong relationships with community resources to facilitate appropriate referrals and support services for residents.
- Address and resolve resident and family concerns, promoting open communication and satisfaction.
Qualifications & Experience
- Master’s Degree in Social Work (MSW) from an accredited program required.
- Current New Jersey license as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- A minimum of 3‑5 years of direct social work experience in a skilled nursing facility or long‑term care setting.
