Position: HOMECARE REFERRAL COORDINATOR

Your role as a Homecare Referral Coordinator

As a Homecare Referral Coordinator at the Korean Women's Association, you will play a crucial role in the centralized intake, tracking, and follow-up of all homecare referrals received from diverse sources. Your responsibilities will include ensuring timely, professional, and coordinated responses to both referral sources and prospective clients, which directly contributes to increasing conversion rates and improving service access. This position demands meticulous attention to detail, as you will be responsible for maintaining accurate and comprehensive records of all referral activity.

Collaboration with regional teams is essential to ensure that no opportunity is missed, allowing us to enhance patient/client advocacy and streamline case management processes effectively.

Requirements for this Homecare Referral Coordinator job

To thrive as a Homecare Referral Coordinator at the Korean Women's Association, you will need a robust skill set tailored to the nuances of the healthcare and homecare landscape. Experience with homecare referral systems and homecare contracting specific to Washington State is essential for navigating the complexities of service delivery. A solid understanding of Long-Term Services and Supports (LTSS) and community-based care programs will enhance your ability to provide efficient case management and intake coordination.

An associate degree in healthcare administration or social services is required, with a bachelor’s degree preferred to deepen your expertise in this field. Additionally, two years of experience in healthcare, homecare, or case management intake coordination will equip you with the problem‑solving skills necessary to respond effectively to referrals, ensuring that clients receive the quality care they deserve.

Your empathetic nature and commitment to customer service will be integral to your success in this role.

Knowledge and skills required for the position are:

  • Experience with homecare referral systems and homecare contracting in Washington State;
    Knowledge of Long-Term Services and Supports (LTSS) and community-based care programs.
  • Associate degree in healthcare administration or related field;
    Bachelor’s preferred. Two (2) years of experience in healthcare or homecare.
  • Case management intake coordination experience preferred.
