LTSS Service Coordinator - Clinician

Location: Field‑based position. Candidates must reside in Lubbock, Hale, Palo Pinto, or Wichita Falls.

Field: This role requires field work to client sites or designated locations, with occasional office attendance for meetings or training. Flexibility and responsiveness to client needs, and direct engagement are essential.

  • Assist the responsible RN in identifying members for high‑risk complications and gather clinical data as directed.
  • Support the RN in determining members who would benefit from alternative levels of care or waiver programs.
  • Provide collected information to the RN for verification, interpretation, and additional assessment as needed.
  • Collaborate in coordinating care for members with chronic illnesses, comorbidities, and/or disabilities to ensure cost‑effective and efficient utilization of health benefits.
  • Apply clinical guidelines and disease‑process knowledge to assist in care‑plan development, monitoring, evaluation, and revision.
Minimum Requirements
  • Hold an LPN/LVN, LSW, LCSW, LMSW, or another relevant license in accordance with state law and a Nursing Diploma or AS in Nursing or related field.
  • Have a minimum of two years of experience working with individuals with chronic illnesses, comorbidities, and/or disabilities in a Service Coordinator or similar role.
  • Be licensed in the applicable state(s) with unrestricted status.
  • State‑specified certification may be required based on contract or law.
Preferred Skills, Capabilities, and Experiences
  • MA/MS in Health/Nursing preferred.
  • Willingness and ability to travel to worksite and other locations as needed.
