Job overviewThe post holder will have a specialist clinical role with responsibility for planning, managing and coordinating the care of people with long term conditions within a defined caseload. The postholder will use case management techniques to reduce unplanned hospital admissions. Inherent in the role is the requirement for the post holder to undertake training to develop skills in medicines management, care co-ordination, managing cognitive impairment, supporting self-care, management and leadership skills in line with the Department of health competency framework for Case Managers.

We currently have vacancy for a band 7 position heart failure nurse specialist with full support and training available for this rotational post between community and acute heart failure teams
We are seeking an innovative, highly motivated forward thinking nurse who has the ability to manage a busy caseload on a daily basis using expert clinical knowledge and judgement whilst providing support to staff. You will be expected to have both a ‘hands on' approach in providing high quality care and have a visible and approachable presence on the wards for inpatient reviews also managing a clinic caseload
The successful candidate will be supported by the ward manager and matron.
