Overview
This post is to facilitate early discharge and prevent avoidable readmission to hospital for children with acute or chronic health care needs. It provides specialist support from an autonomous practitioner to the child’s family or carers, enabling seamless transition from hospital to home through multi-professional and multi-agency collaboration across health care settings.
Responsibilities- Clinical
- Undertake community assessments in partnership with the child and family when assessing needs and planning care.
- Assess, treat, evaluate and manage the care of children with various conditions.
- Recognize and monitor acute exacerbations and initiate appropriate nursing interventions prior to senior clinical input when required.
- Discuss cases with designated senior team members and reassess care needs and outcomes with families/caregivers.
- Provide support to families to enable them to meet their child’s health care needs at home.
- Act as an expert resource to the child and family in the home setting.
- Collaborate with the multidisciplinary team and other service providers to ensure patient-centered, efficient and effective care.
- Contribute to health education/promotion for children, families and carers under guidance from a senior team member.
- Maintain precise documentation in line with professional standards and provide required statistics to management.
- Serve as a role model and resource/advisor to other professionals within the trust and primary health care team.
- Maintain clinical expertise and develop research-based practice within the scope of professional practice, accessing relevant training as appropriate.
- Be conversant with safeguarding procedures and information within the Lancashire safeguarding arena.
- Professional
- Maintain active status on the NMC register and act in accordance with the NMC Code of Conduct and guidance documents.
- Keep skills, knowledge and professional portfolio up to date; participate in ongoing professional development and review processes.
- Maintain legible, accurate records and ensure effective communication/reporting systems in line with NMC standards.
- Follow trust policies and promote quality assurance initiatives; raise the profile of the service through good communication and public relations.
- Attend mandatory training and team meetings as required.
- Management
- Manage a caseload of children with guidance from a senior team member.
- Maintain effective communication with General Practitioners, Health Visitors, District and School Nurses, regional nurse specialists, hospital professionals, patients and families; liaise with other agencies as appropriate.
- Be familiar with statutory and voluntary services available to families with sick children.
- Participate in quality assurance initiatives and support the team leader in resource management.
- Monitor and maintain equipment used by the Community Children’s Nursing service.
- Involve in orientation, induction and supervision of junior staff; ensure patient confidentiality at all times.
- Advise clients on complaint procedures and participate in investigations when necessary.
- Participate in audits of practice and consider user views; seek guidance on matters beyond own knowledge.
- Teaching
- Assist in ensuring that learning environments comply with NMC and university requirements.
- Support, mentor and assess learners at community placements; comply with the trust’s training strategy.
- Health and Safety
- Comply with safety instructions and use equipment properly; report hazards and defects promptly.
- Do not misuse or interfere with health and safety provisions; report accidents or untoward occurrences and complete incident forms as required.
- Engage with children, families and carers to prevent and resolve complaints proactively.
- Standards of Conduct
- Ensure personal actions comply with Trust policies and procedures, including Health and Safety, Equal Opportunities, Confidentiality and Data Protection.
- Promote and safeguard the wellbeing of children in line with The Children Act 1989 and Working Together under the Children Act 1989.
