Community Staff Nurse (part-time) NHS AfC:Band 5 Job overview
We are looking for a dedicated and forward‑thinking senior band 5 registered nurse to join our new self‑care team. If you have the ability to engage staff in supporting our patients on their self‑care journey, you would be perfect for our team.
Main duties of the jobThe successful candidate will have experience of community‑based care, be passionate about patient choices and their abilities to self‑manage and instigate personalised care.
The candidate will work across the Fylde Coast, as part of a community nursing service, undertaking nursing duties in patients’ homes or clinic settings. Supporting patients towards becoming experts in their own health care interventions and providing safe, effective, individualised care to patients and their families.
Your responsibilities will be to support junior staff in delivering a high‑quality service, including oversight of clinical caseloads, clinical supervision, training, facilitation of learning and competence for staff & patients, ensuring confidence and optimisation of skills, and maximising the benefits of self‑management.
The candidate must be willing to undertake training around health coaching, shared decision making and the personalised care agenda, actively promoting a self‑care model whilst ensuring the core clinical skills required.
Utilising support services throughout Fylde Coast, the candidate will support the provision of personalised high‑quality care, ensuring better patient outcomes and positively influencing service implementation and change.
Person specification Education and Qualification- Registered Nurse level 1
- Current NMC registration
- Evidence of ongoing professional development
- Supervisor/Assessor competence or willingness to undertake
- Additional Nursing Qualification relevant to continence
- Able to deal with difficult situations
- Ability to identify and assess patients’ needs
- Ability to use resources appropriately
- Multidisciplinary working
- Willingness to undertake further training specific to role
- Good team worker
- Ability to organise self and others by prioritising and delegating as needed
- Ability to educate others
- IT literate
