Position Summary
We need a 2x permanent, Full-time, Social Workers to work across North Lancashire (e.g., Lancaster, Morecambe & Heysham) as part of our Adult Services Older Adults and Physical Disability Community Teams.

Responsibilities- Undertake strength‑based, outcome‑focussed assessments and support planning, ensuring that the residents of Lancashire receive the right amount of support.
- Support people to access local services, maintain independence, promote health and wellbeing and live independently.
- Work alongside other agencies, professionals and informal carers, providing a holistic service.
- Collaborate with supportive managers and pursue various training and progression opportunities.
- Supportive and Innovative – blend of supportive and innovative thinking, coming up with fresh ideas to achieve the best outcomes for people.
- Empathy – compassion and empathy to support people, listening to what they need, whilst ensuring they feel valued and respected.
